デジタル青モノクロ画像により網膜内層の菲薄化を検出できた陳旧性腎性網膜症の1例

Attenuation of inner retinal layer detected by blue monochromatic imaging by fundus photography in a case of protracted renal retinopathy

要約　目的：デジタルカラー眼底写真に含まれる青成分の画像解析で，網膜内層の菲薄化が検出された陳旧性腎性網膜症の報告。症例：23歳男性が両眼の視力障害で受診した。矯正視力は左右とも0.1であった。網膜浮腫と軟性白斑が多発していた。内科で高血圧と腎不全と診断され，透析が開始された。経過：全身所見の改善に伴い眼底病変は消失し，9年後の視力は左右とも1.2以上に改善した。改善後のデジタルカラー眼底写真から作成した青モノクロ画像で，中心窩付近に散在する低輝度斑があり，光干渉断層計でこれに相当する部位に網膜内層の菲薄化があった。結論：デジタルカラー眼底写真の青成分の画像解析で，過去の網膜虚血による網膜内層の菲薄化が検出できた。

Abstract.　Purpose：To report attenuated inner retinal layer detected by blue monochromatic imaging by fundus photography in a case of protracted renal retinopathy. Case：A 23-year-old male presented with impaired vision in both eyes. Corrected visual acuity was 0.1 in either eye. Both eyes showed retinal edema and multiple soft exudates. He was diagnosed with renal failure with systemic hypertension and started receiving hemodialysis. Findings：Fundus lesions normalized after improved systemic findings. Visual acuity improved to 1.2 or over in both eyes. Ten years after the initial event,digitalized fundus photographs processed by enhancement of blue element showed multiple spots of low luminance. Optical coherence tomography showed attenuation of inner retinal layer corresponding to these spots. Conclusion：Digitalized fundus photographs processed by enhancement of blue element showed attenuation of inner retinal layer.
