A case of craniopharyngioma of 14 years histoy in a 48 years old man was treated with intratumoral injection of radioactive gold of 15 me in the cystic cavity, which revealed sufficient effect of arresting reexpansion of the cyst. Prior to the radioactive treatment, the patient underwent five craniotomies within the last 14 years' period.

1) To treat of a craniopharyngiom which was not to be removed totally, several attempts were applied ; namely intermittent aspiration of cystic content, irradiation, treatment with radioactive iso-tope injected into the cystic cavity or with tissue fixing agent.

2) Radioactive isotope injection into the cystic cavity is a treatment of the choice at present, for inoperable craniopharyngioma.

3) 32P, pure beta emitter, should be applicable for unilocular cyst but for a craniopharyngioma of multilocular cysts with partly solid proliferation, 198Au, containing γ emitter, is more effective than 32P.

4) Prior to the injection of radioisotope into the cyst, it is necessary to examine no leakage of in-jected radioisotope into chiasmal cistern.

5) It is safer and more convenient to inject radioisotope through a previously placed Ommaya reservoir than stereotaxic injection.

6) Fifteen to twenty millicuries of 198Au are sufficient dosis for the initial injection.
