急性心筋梗塞後の再発性虚血発作—その臨床像と発作時ST偏位の意義

Recurrent Ischemia after Acute Myocardial Infarction:Clinical profile and significance of electrocardiographic ST changes during the attacks

　急性心筋梗塞（AMI）後の再発性虚血発作の臨床像を発作時心電図上のST変化と関連づけて明らかにする．AMI 301例を入院中に虚血発作があったもの（虚血群）となかったもの（非虚血群）に分類した．7例はいずれにも分類し得ないため対象より除外した．虚血群を発作時心電図上のST変化よりST上昇群とST下降群に分類した．虚血群（n＝56）では非虚血群（n＝238）より狭心症あるいは心筋梗塞の既往と非Q波梗塞が多かった．虚血群の中でST下降型（n＝26）はST上昇型（n＝29）に比べて多枝病変例が多かった．心筋梗塞の発生には差はなかったが，ST下降群に心臓死が多い傾向を認めた．AMI後の再発性虚血発作のうち，発作時心電図上でST下降を示すものは多枝病変を背景とするポンプ失調例が多く，死亡率が高いため，積極的な血行再建を必要とする．

This study investigated the clinical characteristics of acute myocardial infarction (AMI) complicated by recurrent ischemia, especially relating to the electrocar-diographic ST changes during the attacks. Fifty-six patients with AMI were complicated by recurrent is-chemia (ischemia growp), and 238 were not (non-is-chemia group). The ischemia group was preceded by prior episodes of angina or myocardial infarction in 88%, as compared with 65% in the non-ischemia group (p＜0.05). There were non Q wave infarction in 45% of the ischemia group, and 24% of the non-ischemia group (p＜0.05).
