Electrophysiological study was done in a 28-year-old female who manifested wide QRS complex during recurrent tachycardia and no evidence of Wolff-Parkinson-White syndrome during sinus rhythm. Two atrioventricular accessory conduction pathways (ACPs) were diagnosed ; right lateral ACP with long conduction time and left lateral concealed ACP.

The patient underwent simultaneous surgical interruptions of the both two ACPs successfully. Epicardial excitation was studied during the surgery.
