Gas chromatographyによるCO肺拡散能力測定に関する研究—第1報 insoluble inert gasにneonを用いたsingle breath methodによる健康人CO肺拡散能力について

Studies on DLCO Measurements with Gas chromatography with Reference to Use of Neon as Insoluble Inert Gas.

はじめに

　肺の機能のうちで最も本質的なものであるガス交換の生理を論ずる上において，肺胞におけるガス拡散は極めて重要な問題である。

　1915年Krogh^1）は肺のガス拡散能力について一酸化炭素を用いて初めて測定し発表したが，当時一酸化炭素の簡単な分析装置がなかったためその研究はその後，長い間顧りみられなかった。また1951年にはRiley^2）3）らが酸素肺拡故能力測定法を報告して種々の疾患の肺拡散能力測定に応用されたが，本法は測定の理論的な基礎にある仮定に問題^4）がある上に操作も複雑なことなどから臨床検査としては広く用いられるにいたらなかった。ところが1954年Forster^5）らがKroghの一回呼吸法による肺拡散能力測定法を改良し，さらに赤外線CO分析装置を一酸化炭素の分析に応用するにおよんで，肺胞におけるガス拡故能力の測定が臨床的に応用しうるようになった。

In this paper a technique using the gas chromatograph as an analytical tool for measuring pulmonary diffusing capacity (D_LCO) was presented.

Neon was used as the insoluble inert gas.

Measurements of D_LCO were made on eight healthy subjects by single breath technique. The mean D_LCO was 29.9ml/min/mmHg. These results were comparable to values in the literature reported by other workers, who had used the established methods of analysis.

The use of gas chromatography as the method of gasanalysis and neon as the in-solubie inert gas is very recommendable for measurement of pulmonary diffusing capacity by single breath technique because of its accuracy, rapidness and simplicity.
