 Duodenal cancer is a rare disease; however, submucosal invasion is associated with a high risk of lymph node metastasis. With advances in endoscopic diagnostic and therapeutic techniques and increasing awareness, the detection rate of duodenal cancer has been improving. Duodenal carcinogenesis is thought to follow the adenoma-carcinoma sequence. Familial adenomatous polyposis (FAP) is a well-known high-risk condition, although sporadic cases without a genetic background are also common. Early-stage duodenal cancer is often asymptomatic and is frequently detected incidentally during screening esophagogastroduodenoscopy. Because the duodenum carries a high risk for endoscopic treatment and surgical management often requires pancreaticoduodenectomy, early detection is crucial. However, the endoscopic detection rate of superficial non-ampullary duodenal epithelial tumors (SNADET) during screening remains low. A thorough understanding of duodenum-specific anatomy, endoscopic maneuverability, and scope characteristics is essential to improve diagnostic accuracy.
