小腸潰瘍の鑑別診断―内視鏡診断を中心に

Endoscopic Diagnosis for Small Intestinal Ulcers

要旨　バルーン内視鏡の登場で，小腸潰瘍の詳細観察や組織生検も可能になった．小腸潰瘍には打ち抜き潰瘍や縦走潰瘍，輪状潰瘍，地図状潰瘍など，さまざまな形態があり，病変分布も多様である．潰瘍の発生部位について，腸間膜付着側との関係が重要な情報となる場合がある．しかし，特徴的な内視鏡検査所見を伴う一部の疾患を除いて，小腸潰瘍の原因を内視鏡検査所見のみから診断することは困難である．背景疾患や服薬歴を含む病歴，血液検査，各種画像検査，病理組織学的検査，細菌学的検査，ステロイド薬治療や栄養療法に対する治療反応性も含め，総合的に判断する必要がある．本稿では，小腸潰瘍の内視鏡的診断について，多数の画像を示しながら解説する．

　Balloon assisted endoscopy enabled endoscopic observation and biopsy for small intestinal ulcers. There are various types about shape and distribution of small intestinal ulcers. The location of lesions with regard to mesenteric side or anti-mesenteric side of the small intestine is important information for diagnosis. Except for some diseases with characteristic endoscopic findings, it is difficult to diagnosis by endoscopic findings only. It is important for accurate diagnosis to put background disease, drug history, past and present history, laboratory data, another imaging modalities, histological findings, bacterial examination and response to medications. We described about endoscopic diagnosis for small intestinal ulcers along with many pictures.
