The Evolution and Future Directions of Anesthesiology Education in Japan：A Next-Generation Perspective

はじめに

　教えることや育てることは，教えられる側だけでなく教える側にも学びや気づきをもたらす。医学教育・医療者教育の範囲は知識や技能にとどまらず，人格形成や社会的役割の探究まで幅広い。医育機関では指導が不可欠であるにもかかわらず，効果的な指導方法を学ぶ機会は限られており，“後輩指導は面倒” “何をどう教えたらいいのかわからない” という声も聞かれる。また，若手が学生や研修医指導を行うのは時期尚早と考える指導医もいるが，教え学ぶ関係は年次に関係なく発生するものである。

　わが国において，麻酔科教育は個人や施設のレベルでは発展しているものの，学術的に十分確立されているとは言いがたい。しかし，教育にも理論や科学がある。すでに確立されている教育学的知見を生かし，また日本での教育実践も学術的な視点から見直しながら，麻酔科教育を議論していくことが必要である。

　著者の鵜沼は名古屋大学主催の “現場で働く教員・指導医のための医学教育学プログラム―基礎編―” を修了し，衛藤はハワイ大学留学中に医学教育のフェローシップを修了した。日本と米国で医学教育を学んだ著者らが，麻酔科教育の歴史的背景から現在の潮流までを概観し，グローバルな視点と日本の文脈を考慮した麻酔科教育の新たな方向性を探る。

　We review the historical development of anesthesiology education both in Japan and the United State from the perspective of U40 members who have studied medical education. Approaches used in Japan and the United States are compared, and we propose future directions for anesthesiology education in Japan. Anesthesiology education globally has evolved from apprenticeship models to modern competency-based medical education（CBME）and entrustable professional activities（EPAs）；however academic discussions regarding educational methodologies in Japan lag behind international standards. Although Japan has consistently developed highly skilled anesthesiologists, there is a need to articulate educational goals and methods more explicitly. We propose adopting a “glocal”（i.e., both global and local）approach incorporating international best practices while adapting to Japan’s context. This includes developing Japan-specific competencies and EPAs, applying qualitative research methods to identify existing strengths and improve anesthesiology education, and providing formal training for educators. The integration of educational theories and social sciences is emphasized as crucial for advancing anesthesiology education.

　This article aims to stimulate and promote academic discussions and research on anesthesiology education in Japan. Its goal is to enhance the quality and safety of anesthesia care while maintaining the strengths of Japanese anesthesiology practice.
