 Interest in subspecialty career development is growing among young anesthesiologists. The U40 generation is transitioning from junior to fellow positions in which they are increasingly expected to play key roles in clinical practice and education. Subspecialty training offers a structured path to further career development. This paper focuses on pediatric anesthesia, outlining the history and current status of its subspecialty education in Japan. We also highlight the real-world career development of pediatric anesthesiologists at both university hospitals and dedicated pediatric centers, and we discuss future prospects for subspecialty training. In the United States and Europe, formal fellowship programs requiring specific case experience have been established. In contrast, Japan lacks a comprehensive system, and practical learning often occurs in settings with fewer available cases. Despite this, pediatric anesthesia specialists at Japan’s university and pediatric hospitals are steadily building their careers through diverse case experiences. Japan faces challenges such as a declining birth rate and the increasing centralization of cases. Simulation-based education and the development of educational networks are essential for the future of pediatric anesthesia training. The U40-Working Group aims to promote the sharing of knowledge and support subspecialty career development while working towards improving educational systems.
