緒言とまとめ：『麻酔』創刊号に見る麻酔草創期の麻酔安全性

History and Future Perspective for Anesthesia Safety Increased by Improvements of Drugs, Equipment and Technology, as well as Organizational Reform：Preface and Comments：Patient Safety during Anesthesia in Japan at the Time of the First Issue of MASUI

　『麻酔』第72巻第5号では，麻酔における医療安全の歴史を特集として企画した。日本麻酔科学会が設立された1954年よりも2年早い1952年（昭和27年）に発刊された本誌が，70年以上にわたり読者である麻酔科医と共有してきた総説，特集，症例報告，基礎研究，臨床研究結果などの情報は，まさに，麻酔医療の安全と質向上を目指した歴史そのものである。そして，これは今後も変わることはないであろう。『麻酔』第1巻第1号（図）の記事からは，このころの麻酔の安全性と安全性向上への意識が読み取れる。まず，東京大学第2外科福田　保教授が書かれた巻頭言は，“（前略）大手術や難手術が，近年手軽にしかも成績よく敢行されるようになったことは，ペニシリンのような抗菌剤の発見や（中略），麻酔学の進歩に帰せなければならぬことも見逃されない事実である” と述べたうえで，“手術中の疼痛は勿論，患者の生体管理を全部引き受けてくれる麻酔専門医を吾々は持ち合せがない” と，日本でも麻酔専門医育成の必要性を訴えている^1）。文末には，“今日の麻酔学は吾々メスを持つ者を中心として各方面の関心が必要であって，麻酔薬について製剤や薬理の専門的智識が必要であると同時に生化学，生理学，血清学などの基礎学の力も重要であり，呼吸器循環器代謝などについて内科方面の智識も大切である。（中略）このようにして麻酔学の与る範囲は広い部門によってすすめられなければならない，ここに本誌の活躍が期待されるわけである” とあり，2023年の今日にまさに麻酔医療の安全性と質向上のために再認識すべき点である。

　Our predecessors in anesthesiology endeavoured to decrease decreare adverse effects of anesthesia. Development of advanced medical equipment and technique combined with safe and effective anesthetics has contributed to improvement of patient safety during anesthesia. I found the origin of these attitudes in the first issue of MASUI published in 1952. Reviewing of the articles may assist your further understanding of the special articles published in the current issue of MASUI highlighting history of anesthesia for improving patient safety.
