腹痛を主訴に来院した孤立性左胃動脈解離の2例

Two cases of isolated left gastric artery dissection presenting with abdominal pain

左胃動脈（left gastric artery：LGA）の孤立性動脈解離（isolated arterial dissection）は，極めてまれな症例であり，医学的考察を含めた報告は数少ない。本疾患は，よくありふれた腹痛が主訴となることが多いが，重篤な合併症をもたらす可能性があるため，早期診断および早期介入が必要とされる。急性腹症を呈する症例の鑑別診断に左胃動脈解離を含めた内臓動脈解離（splanchnic artery dissection）を考慮することが重要である。今回我々は，左胃動脈の孤立性動脈解離を2例経験したので，若干の文献的考察を加えて報告する。

Isolated arterial dissection of the left gastric artery（LGA）is an extremely rare condition with only a few reports including medical considerations. Although this disease is often presented with the usual complaint of abdominal pain, early diagnosis and intervention are needed because of the potential for serious complications. It is important to consider splanchnic artery dissection, including left gastric artery dissection, in the differential diagnosis of patients presenting with acute abdomen. We describe two cases of isolated dissection of the left gastric artery with some discussion of the literature.
