尿路真菌症は比較的まれな疾患であり,画像所見に関する報告も少ない。今回我々は,多量のfungus ballを認めた膀胱カンジダ感染症の症例について,画像上鑑別に苦慮した経験から,若干の文献的考察を加えて報告する。
We report a case of a 57-year-old male who presented with massive Candida fungus balls in the bladder. The patient’s history included transurethral lithotripsy and antibiotic treatment for suspected cystitis three months prior. One month before admission, urinary retention led to CT imaging, revealing air and clot-like structures in the bladder, suggesting emphysematous cystitis. Despite catheterization and antibiotic therapy, the patient later presented with abdominal pain. CT scan indicated bladder perforation, necessitating emergency surgery. Pathology confirmed Candida fungus balls. With its atypical imaging findings, this rare case highlights the importance of considering fungal etiology in unusual bladder pathologies, particularly in patients taking medications that increase the risk of infection, such as sodium-glucose co-transporter-2 inhibitors.
