孤立性総肝動脈解離の1例

A case of isolated common hepatic artery dissection

大動脈解離を伴わない孤立性腹部内臓動脈解離の報告は，近年の画像診断の発達に伴い増加してきている。しかしながら，大半は上腸間膜動脈や腹腔動脈の解離であり，総肝動脈に生じた孤立性腹部内臓動脈解離の報告例はほとんどない^1）。今回我々は，孤立性総肝動脈解離に対し保存的治療を行った1例を経験したので，文献的考察を加えて報告する。

The spontaneous dissection of a visceral artery unrelated to aortic lesion is rare. There is no consensus on optimal treatment strategies. We report a rare case of isolated dissection of the common hepatic artery in a 48-year-old man. The patient was successfully managed by conservative medical therapy with analgesic agent, because the abdominal pain was improved and liver functions were normal. No recurrence has occurred for six months.
