腎部分切除術後のCTで膀胱内に脂肪成分を認め，乳び尿が疑われた1例

A case of chyluria after partial nephrectomy diagnosed with computed tomography

乳び尿は尿路とリンパ管との交通により生じる尿所見である^1）。腎部分切除術後の乳び尿はまれな合併症であり，その多くは無症候性であるが，低アルブミン血症をきたして治療を要した症例も報告されている^2）。今回我々は，腎部分切除後のCTで乳び尿を疑うことができた1例を経験したことから，文献的考察を加えつつ報告する。

We reported a case of 78-year-old male who presented with chyluria after partial nephrectomy. The patient underwent robot-assisted laparoscopic partial nephrectomy for excision of right clear cell renal cell carcinoma. Follow-up CT scans 3 years 10 months after surgery showed a fat-fluid level in the bladder, consistent with chyluria. The patient had no urinary complaints and was treated conservatively. Chyluria after partial nephrectomy is rare, but we found that it may severe. Fat in the bladder suggests the presence of chyluria, and radiologists should recognize this finding.
