Appropriate use guidelines of amyloid PET imaging agents：summary of the 3rd edition and key points of revision

アミロイドPETイメージングは生体におけるアミロイドβ（Aβ）の脳内沈着を非侵襲的に可視化する診断技術である。現在アルツハイマー病（AD）の最早期の病態を検出することが可能なバイオマーカーとしての地位が確立し，病態研究や疾患修飾薬の治験に広く使われている。我が国ではアミロイドPET診断薬の合成装置が薬事承認され，診断薬としての製造販売（デリバリー）も開始されたが，保険収載は見送られてきた。2023年に抗アミロイドβ抗体薬レカネマブがADの進行を抑制する初めての治療薬として承認を受け，この治療薬投与の要否を判断する目的でAβ病理の存在を確認する場合，保険適用が認められることになった。本稿では，これに合わせて改訂された「アミロイドPETイメージング剤の適正使用ガイドライン（改訂第3版）」^1）の概要と改訂の要点について解説する。

The anti-amyloidβ antibody drug lecanemab has been approved for the first time as a treatment to suppress the progression of Alzheimer’s disease. Along with this, amyloid PET, which is used to select patients for treatment, will be covered by insurance and implemented in clinical practice. This article outlines the contents of the revised 3rd edition of the Guidelines for Appropriate Use of Amyloid PET Imaging Agents, which should be referred to when performing amyloid PET in clinical practice.
