アミロイドイメージングは生体におけるアミロイドβ（Aβ）の脳内沈着を非侵襲的に画像化する診断技術である。現在アルツハイマー病（AD）の最早期の病態を検出することが可能なバイオマーカーとしての地位が確立し，病態研究や疾患修飾薬治験に広く使われている。我が国では合成装置が薬事承認され，診断薬としての製造販売も開始されたが，保険収載はまだされていない。しかし限定的ではあるが日常診療での使用に関する評価も進みつつあり，今後疾患修飾薬が実用化すれば広く普及する可能性がある。本稿では，画像病理相関の第Ⅲ相治験を経て効果効能が承認された標題の3剤のアミロイドPET診断薬を診療において用いるうえでの留意点を，適正使用ガイドライン^1）を中心に解説する。アミロイドイメージング技術の概要については最近の総説を参照していただきたい^2）3）。

Three ¹⁸F-labeled amyloid PET diagnostic tracers；¹⁸F-florbetapir, ¹⁸F-flutemtamol, and ¹⁸F-florbetaben, have been approved after clinical trials as diagnostic agents capable of estimating senile plaque density in the living human brain. In introducing this diagnostic technique into daily clinical practice, it is necessary to arrange its application and operation in accordance with the appropriate use guidelines in Japan. This paper will discuss the main points of clinical amyloid PET diagnosis, focusing on the application of the appropriate use guidelines.
