梅毒は梅毒トレポネーマ（Treponema pallidum sub-species pallidum：T. pallidum）を原因とする感染症である。我が国では2010年から報告数が増加，2018年に年間7,000人を超え，一時減少に転じたものの2021年には8,000人程度と近年は増加傾向にある^1）。世界保健機関の報告によると，2020年には世界で700万人が新たに感染したと推定されている^2）。梅毒は複雑かつ多彩な病変を示し，2期梅毒においてはまれに肝炎を引き起こし，早期梅毒性肝炎として知られている。梅毒患者の増加に伴い潜在的に早期梅毒性肝炎に罹患する患者数も増加するものと予想されるが，その一方で当疾患の画像所見を含めた報告は少ない。今回早期梅毒性肝炎に起因すると考えられる画像所見を経験することができたため考察を加えて報告する。

A man in his 40s presented with the chief complaint of poor blood sugar control. Evaluation showed elevated carcinoembryonic antigen（CEA）and carbohydrate antigen 19-9（CA19-9）levels. Three months later, he developed widespread expansion of erythema and papules mainly on the extremities, and was found to be positive for Treponema pallidum antibodies and rapid plasma reagin（RPR）tests based on which he was diagnosed with syphilis infection. In addition, biochemical examinations showed elevated biliary enzymes, and computed tomography and magnetic resonance images showed a diffuse mass lesion in the liver, necessitating differentiation between intrahepatic cholangiocarcinoma and inflammatory lesions. Liver biopsy showed inflammatory cell infiltration around the interlobular bile ducts, consistent with syphilitic hepatitis. He was finally diagnosed with early syphilitic hepatitis due to rapid reduction of the liver mass with antibiotic treatment, improvement of biliary enzymes, reduction of intrahepatic mass lesions, and normalization of tumor markers.
