肉腫と類似した像を呈した子宮腺筋腫の1例

A case of uterine adenomyoma mimicking sarcoma

子宮腺筋腫（adenomyoma）は腺筋症病巣が子宮筋層内に腫瘤を形成した状態であり，病巣の形態もポリープ状や漿膜下に進展した場合には卵巣腫瘤に類似するものなど多彩である^1-4）。今回我々は，MRIで子宮に肉腫に類似した腫瘤を認め術後早期に再増大をきたした子宮腺筋腫を経験したので報告する。

Uterine adenomyoma is a condition in which adenomyosis of the uterus results in the formation of a mass. Cases with polyp-like and cystic forms have been reported. We report a case of uterine adenomyoma that had a relatively smooth border, but bulged from the uterine cavity beyond the cervix, progressing beyond the uterine myometrium to the subserosa. Rapid recurrence was seen after surgery, showing a course similar to that of malignant tumors such as sarcoma. The tumor formed a hemorrhagic cyst, representing a useful finding for diagnosing uterine adenomyoma. The patient was a premenopausal woman, and histological examination showed an abundance of estrogen receptors, presumably causing the recurrence and rapid growth of the tumor.
