晩発性皮膚ポルフィリン症（porphyria cutanea tarda：PCT）は飲酒やウイルス性肝炎などによりヘム合成過程の障害が生じ肝機能低下や皮膚の光線過敏症を起こす疾患で，尿中ウロポルフィリンの上昇により診断される。これまで本疾患では肝に結節状の限局性脂肪沈着を呈し，それには鉄沈着も関与するという報告があることから^1），今回，鉄および脂肪の分布を中心とした画像所見について検討した。

Porphyria cutanea tarda（PCT）is a disease in which the heme biosynthesis is impaired due to alcohol or viral hepatitis, resulting in liver dysfunction and photosensitivity. Multifocal hepatic echogenic areas have been reported in patients with PCT. By means of IDEAL-IQ sequence, we found both fat and iron is heterogeneously increased in the liver in 5 PCT patients；more fat in the echogenic area, and more iron in the background liver. This “fat-iron reversed pattern” was suggested to be characteristic to PCT.
