エピドラスコピー（硬膜外腔内視鏡）は，難治性腰下肢痛患者の硬膜外腔を直接観察し，同時に効果的な疼痛治療を行うことのできる画期的な方法である．硬膜外腔の肉眼的観察によって，通常の画像診断では描出しにくい病態，すなわち硬膜外腔や神経周囲の炎症や癒着などの情報を得ることができる．これらの内視鏡所見は，病歴，理学所見，X線所見などと対比して評価することが大切である．今後，エピドラスコピーは消化管内視鏡と同じように，腰下肢痛の内視鏡診断・治療においてさらに重要な位置づけを占めるようになると考えられる．
Epiduroscopy is a new technique for diagnosis and treatment of chronic low back pain. In this paper, we de-scribed the normal and abnormal findings in the epidural space in patients with low back and leg pain. Epiduroscopy in such patients revealed inflammation, adhesions and fibrosis around the involved neural tissues, which were difficult to be detected by usual radiographical findings. The abnormalities in the epiduroscopic find-ings reflect various stages of impaired pathological processes.
