　近年，硬膜外腔鏡下癒着剥離術（エピドラスコピー）は，慢性腰下肢痛患者の従来の治療法と異なり，診断と治療の両面を持ち合わせた治療方法としてその有用性が報告されている．その治療意義は，患者の疼痛の病因となっていると思われる硬膜外腔または神経根周囲の癒着組織を剥離することにある．直径0.9mmファイブロプティック内視鏡と先の鈍なガイドカテーテルの開発により，エピドラスコピーは比較的安全な手技として確立しつつある．しかしながら，基本的な術前検査や解剖の知識はこの手術の安全性を保つためには必要不可欠なものである．本稿では，どのような術前検査が必要か，そしてエピドラスコピーに必要な解剖について概説する．

Recently, epiduroscopic adhesiolysis has been reported to offer both diagnostic and therapeutic advantages compared with conservative treatment for the low back and limb pain patients. The therapeutic meaning of this procedure is the adhesiolysis of scar tissue in the epidural space or around the affected nerve root correspond-ing to patients' symptoms. Although the invention of flexible, 0.9 mm fiberoptic endoscope and round tip guide-catheter that enabled this procedure to be a safe method, basic understanding of pre-surgical examination and anatomical structure is essential for the safety of this procedure.
