昔からわれわれ医療人にとって，硬膜内・外を観察するのは夢であった．ときを経て機器・技術の進歩で現在ではそれが可能になった．外科的治療を受けた腰痛患者の約30％は数年以内に同じ痛みを感じ，椎間板ヘルニアの患者の85％は適切な保存的治療で3〜6か月でその症状が改善している．この両者の中間的な治療を行うエピドラスコピー（硬膜外腔内視鏡）は腰痛治療には有用である．硬膜外腔の浮腫・炎症，癒着，血管状態を直接観察できることは，癒着剝離，薬剤の最適部位への投与，炎症性サイトカインの洗浄効果などが得られ，患者のQOLを向上させる．エピドラスコピーを通して脊柱管内でいろいろ操作できればもっとすばらしい結果が将来得られるだろうし，そうなってほしいと思っている．
Physicians have long been wanting to see the interthecal and epidural space directly. Recently, the improve-ment of technology and instruments have made us possible to observe the above mentioned space.
Approximately 30% of patients who underwent surgical procedure for chronic back pain are suffering from the same symptoms they had before the surgery. Eighty percent of patients with fresh interdisc herniation re-ceived conservative therapy and are satisfied by their improved symptoms within 3 to 6 month.
