胸腔鏡下手術のクリニカルパス—自然気胸の肺切除を中心として

Clinical Paths on Thoracoscopic Surgery in Pneumothorax Patients

　自然気胸は年齢，性別により原因が異なる疾患である．今回呈示したものは若年者を対象としている．気胸の患者は精神面の訴えが多い傾向にあるので，これを考慮に入れたクリニカルパスを作ることが重要である．当院のクリニカルパスの特徴を以下に挙げた．ドレーンは自己管理としているので，医療側のドレーン状況の把握は重要な項目として扱っている．書き込みができるようにスペースを十分にとることで，柔軟性を持たせている．さらに“いたわりの言葉”が毎日のチェック項目として挙げられている．QOLを重視した内視鏡治療では患者の精神面でのケアも考慮したためである，言葉によるQOLの向上を目的としたものである．

The clinical paths are for young patients. It is important to take mental care into consideration to make the better form of clinical paths because pneumothorax patients are generally nervous. The characteristic of the clinical paths is as follows. Medical staff need to check every day if a drainage tube is working properly, because the patients must control the drainage tube by themselves as a rule of surgery department. They actually con-nect and release the drainage tube to a suction device with clamps by themselves.
