気胸を起こす嚢胞性肺疾患は，胸腔鏡下手術後も再発を繰り返しやすい．気胸患者は常に4つの不安を抱えている．すなわち呼吸苦や胸痛，健側肺の気胸，治療効果，手術そのものに対する不安である．これらを全体として解決することが気胸治療として重要である．胸腔鏡でブラを切除するだけでは不十分であり，再発を繰り返しやすく，付加的治療が必要である．筆者が考案したCOVERING法は，臓側胸膜を肥厚させ壁側との癒着は起こさず再発を予防できる優れた方法である．再生酸化セルロースメッシュが最も適した材質である．気漏の程度分類は臨床的に有用である．女性気胸，若年者気胸の術後再発が多いので，今後，研究されるべき分野であろう．
Bullous lung diseases often cause recurrent pneumothorax. Even if the operation was performed, there is a possibility of postoperative recurrence to some extent. It is difficult to declare what a therapy for pneumothorax and bullous lung diseases are because it is not enough just to cut away the bullae. The patients should be treated mentally, physically and less invasively. The 4 anxieties should be solved for pneumothorax patients. Four anxie-ties are for symptom, the normal side of lung, the effect after therapy and surgery.
