自然気胸に対する胸腔鏡下手術の実際

Practical techniques in thoracoscopic surgery for spontaneous pneumothorax.

　これまでに経験した自然気胸に対する胸腔鏡下手術の手技の要点を記述した．自然気胸は良性疾患であり，胸腔鏡手術のよい適応であるが，特に安全に注意して手術を行わなければならない．手術は麻酔医の協力の下，穿刺下ないしは小開胸下にブラをendo staplerで切除する．癒着剥離，縫合術やフィブリン糊の新しい使用法は有力な手段である．また，出血やendostaplerなどのトラブルシュートについても述べた．

Since spontanous pneumothorax is a good indication for thoracoscopic surgey, the procedure should be done safely. The operation is performed under the manage-ment of anesthesiologist. Blunt wound or small tho-racotomy is made, bulla is resected with endo staplers. Release of adhesions, suturing, and new method of applying fibrin glue are useful techniques. Trouble shooting for sudden bleeding or endo stapler trouble are also described.
