自然気胸手術の術中所見と病理所見の比較

Comparative studies on the operative findings and microscopic appearance of spontaneous pneumothorax

　2年間に自然気胸にて手術を施行した162例の術中所見，病理所見を解析した．再発数は162例中7例（4.3％），無再発数は155例であった．切除線未処置群0％（0／15例），フィブリン糊群10.4％（5／48例），フィブリン糊＋ポリグリコール酸（以下，PGA）フェルト群2.0％（2／99例）であった．再発原因は切除線のブラ新生5例，見逃し2例であった．病理所見では切除線再発群で壁の線維性肥厚が100％（5／5例），ブレブの存在が80％（4／5例）であった．術中所見では大畑分類Ⅱ型が無再発群で68.4％（106／155例），切除線再発群で100％（5／5例）が特徴的であった．大畑Ⅱ型で広範囲に広がるブラの近傍には肉眼的には確認し得ない病理学的ブレブが存在している可能性があり，この存在が切除断端の再発に関与している可能性がある．

The purpose of this study is to compare the operative findings and microscopic appearance of spontaneous pneumothorax and to predict high risk groups of recurrence after VATS. We performed 162 operations for spontaneous pneumothorax from 2000.1 through 2003.12. We experienced 7 cases of recurrence, of which 5 cases were due to new bulla or bleb formation near the staple line of first operation, and 2 cases were due to overlook-ing of the bulla.
