中高齢者自然気胸に対する胸腔鏡下手術の有用性

Thoracoscopic Operation for Spontaneous Pneumothorax in Middle-aged and Elderly People

　1996年8月から2001年4月までの，50歳以上の中高齢者自然気胸10例に対し，胸腔鏡下肺部分切除術（以下，胸腔鏡下手術）を5例，開胸肺部分切除術（以下，開胸手術）を5例（6側）施行した．胸腔鏡下手術例の平均年齢は69.8歳，平均手術時間は79.4分，術後平均在院日数は16日，術後気管内挿管例は2例であった．開胸手術例の平均年齢は69.4歳，平均手術時間は100分，術後平均在院日数は27.3日，術後気管内挿管例は4例，そのうち3例に気管切開を必要とした．胸腔鏡下手術は，開胸手術に比べ手術時間，術中出血量，術後経過，術後在院日数，術後疼痛すべてにおいて良好であり，今後さらに適応を拡大できると考えられた．

Between August 1996 to April 2001, 10 patients with spontaneous pneumothorax were treated at our hospital by thoracoscopic partial lung resection (5 patients) and open partial lung resection (5 patients). All the patients were over 50 years of age. The average age of patients at surgery who underwent thoracoscopic procedure was 69.8 years. The average operation time was 79.4 minutes and the average postoperative hospitalization was 16 days. Postoperative intratracheal intubation was performed on 2 patients. For patients who underwent thorac-otomy, the average age at surgery was 69.4 years.
