自然気胸に対する胸腔鏡下手術—術中合併症，特に自動縫合器のトラブルを中心に

Thoracoscopic Surgery for Spontaneous Pneumothorax: Perioperative Complications, Troubles due to the Autosuture

　自然気胸に対する胸腔鏡下手術の術中合併症，特に自動縫合器のトラブル，発生後の対策，術中に注意するポイントを中心に報告する．われわれの経験した術中合併症，トラブルは，自然気胸71例中手術器具に関するものを6例に（うち自動縫合器による合併症5例—自動縫合器総使用本数の1.7％，トロッカーによるもの1例），術中低酸素症を高齢者の気腫性肺嚢胞の2例に認めたのみであり，ほかの術中合併症は経験していない．自動縫合器によるトラブルの内訳は，（1）ステイプルラインの継ぎ目からの出血，肺瘻，（2）縫合部離開，（3）fire後に自動縫合器が肺実質から外れない，などであった．自動縫合器のトラブルは生命に関わるものがあり，常に速やかに開胸に移行できる態勢を整えておく必要がある．

We report intraoperative troubles, especially troubles due to the autosuture, in thoracoscopic surgery for spontaneous pneumothorax. We experienced 6 cases of intraoperative troubles, 5 cases due to the autosuture (1.7% of total fired autosutures in pneumothorax operation), and 1 case due to the trocar, 2 cases of hypoxia in elder patients of bullous emphysema. Autosuture troubles were, (1) bleeding and air leakage from a joint of the staple line, (2) tearing of the suturing line, (3) inseparable autosuture from lung tissue.
