最後には皮膚に小切開をおくことの多い腹腔鏡下大腸手術は，HALS（hand-assistedlaparoscopic colectomy）のよい適応である．筆者らはこれまでに約30例のHALSによる大腸手術を行ってきた．手技的にはLAC（laparoscopic-assisted colectomy）の場合と同様，大腸の剥離層をよく理解して行えば，安全で迅速な操作が行える．このとき挿入した手は操作鉗子の補助として用い，用手剥離は必要最小限にとどめることが肝要である．腸間膜や血管の処理でも特に術野の展開が容易で，また出血に対して素早く対応できることでの安心感が大きい．HALSはそれのみでも有用な手技であるが，さらに進んでLACへ移行する場合もその移行を円滑にすることができる．今後，大腸手術は腹腔鏡下に行われることがますます多くなると考えられるが，HALSはそのオプションとして非常に有用である．
Most laparoscopic colorectal surgery require a small incision in the abdomen. Hand-assisted laparoscopic surgery (HALS) is feasible for colorectal disease, because a surgeon can insert his hand into the abdomen through the incision. We performed 30 cases of hand-assisted laparoscopic colorectal surgery. HALS is acceptable for many surgeons who have a lot of experience in open surgery, but not in laparoscopic surgery. This technique allows the surgeons to quickly and safely perform such procedures as mobilization and resection of the colon or division and ligation of the vessels.
