HALSによる幽門側胃切除—吊り上げ法と経口長軸サーキュラーステイプラーの応用

Hand-assisted Laparoscopic Distal Gastrectomy: with the Use of Wall-lift and a Per-oral Long-axis Circular Stapler

　複雑な手技を要求される腹腔鏡下胃切除術および再建術を平易化し，かつリンパ節郭清を開腹術と同等に行う手技を検討した．動物実験8例と臨床5例の経験から，腹壁吊り上げ法，hand-assisted laparoscopic surgery（HALS）および経口長軸サーキュラーステイプラー（FLEXISTAP^®）の使用による新しい腹腔鏡下幽門側胃切除術式を考案した．本法はD₂リンパ節郭清と無理のないBillroth Ⅰ法を可能とした．術創は右下腹部に存在することから創痛管理および美容上も優れていると思われる．本術式の基本は，胃全摘あるいは周辺臓器合併切除にまで応用可能と考えられる．

We have attempted to improve laparoscopic gastrectomy which usually requires comlex and difficult procedures including GI reconstruction. Based on eight experiments with pigs and five clinical cases, we have developed hand-assisted laparoscopic distal gastrectomy with the use of wall-lift and a per-oral long-axis circular stapler. The new operation has enabled us to easily perform a rational lymph node dissection and gastroduodenal anastomosis. The small right lower quadrant incision similar to appendectomy is satisfactory from the cosmetic viewpoint and in terms of pain control.
