1．災害医療総論—ICUをおびやかす災害とマスギャザリングを理解し，その対応を考える

Disaster medicine：An overview

災害は，施設の損壊やライフラインの障害，スタッフの不足，患者の増加などによってICUの診療をおびやかす。災害を制御することは難しいが，対応の準備は可能である。

　我が国で準備されている災害医療体制の4つのコンポーネント，すなわち，病院機能，情報共有，医療支援，医療搬送を理解しておき，これを活用できるようにしておきたい。これらは行政によって体制が構築されているものが多いが，学会レベルにおいても日本透析医会や日本集中治療医学会により取り組みが行われている。第32回オリンピック競技大会，東京2020パラリンピック競技大会の開催を控えた今，マスギャザリングという概念の理解，それが及ぼす影響についてのリスク評価と対応計画の策定は集中治療医が取り組むべき課題となっている。

Disasters threaten practice in the ICU by destroying facilities, disrupting lifelines, causing a lack of staff and increased patient admissions. It is difficult to control disasters, but preparations can be made. Understanding the four components of the disaster medical system：hospital functions, information sharing system, medical support team, and medical transport will be helpful. These systems are often organized by the government, but some academic societies （e.g. the Japanese Society of Intensive Care Medicine） are also involved. Understanding the concept of MASS GATHERING, assessing its impact and discussing the plan are important and necessary for proper function of the ICU. The Tokyo 2020 Olympic and Paralympic games will be held in the near future.
