栄養障害は，慢性閉塞性肺疾患（COPD）の代表的な併存症で，予後不良因子である。栄養障害を伴う安定期COPD患者に対する栄養療法は，低体重，呼吸筋力，生活の質（QOL）などを改善することが報告されているが，急性期の栄養療法の有効性を示すエビデンスはほとんどない。一方で，栄養投与は酸素（O₂）消費量・二酸化炭素（CO₂）産生量を増大させるため，特にCO₂が上昇している患者にとって，栄養療法は潜在的なリスクとなる。本稿では，栄養療法に伴う有害事象に焦点を合わせ，CO₂が上昇しているCOPD患者に対して妥当性が高い急性期栄養療法について考察する。

Malnutrition is common in patients with chronic obstructive pulmonary disease （COPD） and is associated with a poorer prognosis and impaired long-term survival. In stable malnourished patients with COPD, nutritional supplementation significantly increases weight and respiratory muscle strength and improves overall health-related quality of life. In acute care settings, however, the evidence for benefits of nutritional therapy in patients with COPD is lacking. Caution is needed to avoid the complications of nutritional treatment. This therapy may increase oxygen consumption and carbon dioxide production. In this article we discuss “good” or “not-so-bad” nutritional treatment in patients with COPD and hypercapnia.
