膠原病や血管炎に呼吸器疾患を高頻度合併することは知られているが，重症急性呼吸不全をきたした患者を診たときに，膠原病・血管炎によるものを想起することは難しい。特に呼吸器症状が初発症状として現れた場合にはなおさらである。これらの呼吸器合併症は，気道病変，肺実質，血管などさまざまな解剖学的部位に異常をきたすが，原因となる疾患により侵される部位が変わってくるため，解剖学的に鑑別を考える。また疾患自体によるものに加え，治療薬によるもの，感染症によるものがあり，これらももれなく鑑別する必要がある。

Autoimmune connective tissue disorders and systemic vasculitis frequently involve the respiratory system, and may present with life-threatening manifestations, requiring intensive care management. Pulmonary involvement sometimes precedes other systemic manifestations. In such situations, intensivists may not consider these diseases as the cause of pulmonary manifestations. All components of the respiratory system, the airway, lung parenchyma, alveolar capillaries, lung vessels, pleura, and ventilator muscles, may be involved in these diseases. Individual autoimmune disorders present with characteristic patterns of respiratory involvement, which may help establish the diagnosis. One should also consider infection-related and drug-induced lung disorders associated with these systemic diseases.
