6．心筋梗塞患者における酸素療法—有益か有害か

Oxygen therapy for acute myocardial infarction

半世紀以上前から，心筋梗塞患者に対して酸素療法は行われてきたが，その有益性を支持する明確な根拠は乏しく，近年では低酸素血症のない患者への酸素療法について有害性を示唆する研究が複数報告されている。その有益性あるいは有害性について，観察研究ながらこれまで多くの報告がなされている。また有害となる可能性の根拠として，酸素投与が冠動脈や血行動態に与える悪影響についての研究も散見される。さらには，より質の高い無作為化比較試験やシステマチックレビューが近年発表された。これらの結果を反映し，近年発表された欧米のガイドラインを見ると，特に低酸素血症のない心筋梗塞患者に対するルーチンの酸素投与は推奨されていない。以上から，現状では酸素飽和度などを指標にして患者への酸素療法を考慮することが妥当と考えられる。

Oxygen therapy for patients with an acute myocardial infarction （AMI） has been used for more than a half century. To justify this common practice, a significant number of laboratory and clinical studies have examined the benefits and harms of oxygen therapy on systemic and coronary hemodynamic status and clinical outcomes. However, a clear benefit of its routine use for patients with an acute AMI has not been confirmed. Recently, several randomized controlled trials and systematic reviews have not shown benefit from the liberal use of oxygen for patients with an AMI or acute coronary syndrome （ACS）. Recent guidelines from Western countries recommend that oxygen therapy should not be given to patients with ACS without low arterial oxygen saturation （SaO₂）. Therefore, administration of oxygen therapy based on SaO₂ is reasonable for these patients.
