Hyperbaric oxygen therapy (HBO) improves a patient's condition in certain disease states by delivering a high concentration of oxygen in a pressurized environment. This article focuses on acute carbon monoxide (CO) poisoning, which is frequently encountered, and decompression illness/air embolism, which is treated with HBO as standard therapy. HBO is not always required for patients with acute CO poisoning, but should be considered in those with severe acute CO poisoning who have loss of consciousness, ischemic cardiac changes, neurological deficits, significant metabolic acidosis, or COHb ≧ 25%. The standard technique for giving HBO therapy to patients with acute CO poisoning has not been established. The most common strategy for treating decompression illness/air embolism is described in the US Navy Treatment Table for Recompression. Since facilities are limited, whether to transfer a patient may bear unique considerations.
