蝶形骨洞原発の乳頭腫

Inverted Papilloma in the Sphenoid Sinus

I．はじめに

　鼻・副鼻腔粘膜に発生する乳頭腫は比較的稀な良性腫瘍とされるが，臨床的には拡人性増殖により骨破壊を伴うことがあり，再発率が高くときに癌化する例がある点で取扱いに注意を要する疾患である。

　鼻・副鼻腔乳頭腫は主に鼻腔，上顎洞，篩骨洞に発生するが，今回蝶形骨洞原発の稀な症例を経験したので報告する。

　An inverted papilloma arising from the right sphenoid sinus was found in a 59-year-old man who complained of progressive nasal obstruction and mucopurulent nasal discharge. A polypoid granular tumor was seen in his right nasal cavity and the epipharynx. Histologic examination of this mass showed typical inverted papilloma with no malignancy. Tumor masses in the nasal and paranasal sinuses were removed by openig the epipharynx via the maxillary sinus.

　Since recurrence was occurred 28 days postoperatively, re-operation was accomplished. However, the patient unfortunately died from fulminant hepatitis 128 days after the operation.

　Because of high recurrent rate of the tumor, complete resection and intensive postoperative fol-low up are required.
