I．はじめに

　蝶形骨洞感染性疾患は比較的まれで，多くは他の副鼻腔炎からの炎症の波及により起こる^2）．一般には耳鼻咽喉科医によって治療されることが多いが，頭痛等の訴えが多く，脳神経外科にて頭蓋底部病変として発見されることもある．われわれは頭蓋底を広範に破壊した蝶形骨洞粘液嚢腫の1例を経験したので報告する．

Infection of the sphenoid sinus is an uncommon clinical entity frequently misdiagnosed because of itsvague symptoms and paucity of clinical findings. We report a case of sphenoid sinus mucocele complicatedby retro-orbital pain and ipsilateral optic atrophy. CT scan and MR image revealed a cystic mass lesion ofthe sphenoid sinus. Bone around the lesion was eroded. The patient underwent transnasal drainage of thecystic mass. We conclude that transnasal drainage is the safest method of treatment of an infectious lesionin and around the sphenoid sinus.
