頭蓋底脊索腫の1例

Cranial Chordoma: Report of a Case

I．はじめに

　脊索腫は比較的稀な悪性腫瘍であるが，咽頭部腫瘤などを初発症状とすることがあるためわれわれ耳鼻咽喉科医も遭遇する可能性があり，またその発生する部位とそれに伴う臨床症状が複雑で組織診も困難であることが少なくない。さらに頭蓋部に発生した脊索腫は解剖学的困難さから完全摘出が難しく，放射線療法や化学療法の効果も少ないため予後の悪い疾患とされてい歓。今回われわれは主にニトロソウレア系抗腫瘍剤ACNU〔1-(4-amino-2-methyl-5-pyrimidinyl) methyl-3-(chloroethyl)-3-nitrosourea hydrochloridc〕の局所動注が症状の改善に有効であったと思われる頭蓋底脊索腫1例を経験したので，その経過と若干の文献的考察とを加え報告する。

　A 51-year-old man complaining of abnormal feeling in the tongue, facial pain and headache was found having cranial chordoma.

　On physic:al examination, a swelling in the pharynx and atrophy of the right side of the tongue caused by paralysis of the right hypoglossal nerve were found. CT scan showed a space-occupying lesion between the skull base and the pharynx.

　The patient was treated with irradiation and anticancer drug (ACNU). Radiotherapy was not successful, but chemotherapy brought partial recovery.
