蝶形骨洞原発癌の2症例

CARCINOMA OF THE SPHENOID SINUS: A REPORT OF TWO CASES

　Ⅰ．緒言

　蝶形骨洞に発生する腫瘍は比較的まれなものであるが，ことに原発悪性腫瘍の報告例は少なく，本邦においては4例^1）-4）を数えるにすぎない。この洞の周囲には容易に損傷を受ける部位があり，洞それ自体の症状というものはほとんどなく，症状はこれら隣接部位と関連したものとなつて現われてくる。したがつて，この洞を侵す悪性腫瘍は一般に診断が遅れるので，原発部位を明確にするのは困難であり，他の頭蓋底附近の腫瘍についても同様であるが，広汎に進展したものでは，剖検によつてだけでは結論を出すことはできない。また逆に，蝶形骨洞原発癌の診断を受けるべきもので，他の疾患とされているものも多いのではないかと推察される。私どもは初発より2年の経過をとり，はじめ頭蓋底附近の腫瘍と考えられ，原発部位を探索するうち，嗅裂にあらわれた腫瘍塊より，蝶形骨洞原発癌の診断がつき，剖検によつて確認された症例と，試験開洞による生検で確かめられた症例を経験したので報告する。

　Two cases of sphenoid sinus carcinoma are reported. In the first case a man, aged 58, complained of swelling of the right side of the neck below the ear, disturbance of speech, dysphagia, and 4 months later, an onset of diplopia, ptosis of the left palpebra and unequal pupil. The patient died 2 years after the first onset of these symptoms. The post mortem examination showed the cancer originating in the sphenoid sinus to have involved the basal region of the cerebrum and spread into a large portion of the pons.

　In the second case a man, aged 52 complained of persistent headache, diplopia and ptosis. A diagnostic operation revealed a cancer in the sphenoid sinus as its original site.
