蝸牛に進展した真珠腫症例

A Cholesteatoma Extended into the Basal Turn of Cochlea

I．はじめに

　真珠腫性中耳炎による内耳炎合併や顔面神経麻痺は日常診療で，しばしば遭遇する疾患である。しかし，蝸牛瘻孔とくに蝸牛内にまで進展した症例は少ない。最近われわれは，二次性後天性真珠腫と考えられる症例で，陳旧性顔面神経麻癖を伴い，蝸牛内に進展し，内耳部分摘出術を施行した症例を経験したので報告する。

　A 32-year-old male who suffered from middle ear cholesteatoma which extended into the vestibule and the basal turn of cochlea was reported. He had suffered hearing impairment of his right ear since childhood. X-ray films and physical examinations revealed destructed labyrinth due to cholesteatoma. Therefore, radical operation of middle ear was performed. Cholesteatoma invaded into the vestibule, semicircular canals and even into the basal turn of cochlea.

　According to several reports, cochlear fistula was rare, and the authors consider that cholesteatoma with intracochlear extension was still rare.
