中耳真珠腫症における耳小骨の変化について

PATHOLOGICAL CHANGES OF MIDDLE EAR OSSICLES DUE TO CHOLESTEATOSIS

　Ⅰ．まえがき

　中耳真珠腫症（Cholesteatosis of middle ear）は真珠腫性中耳炎（Otitis media cholesteatomatosa）あるいは慢性真珠腫性中耳炎，仮性真珠腫，真珠腫（Pseudocholesteatom，Cholesteatom）等多くの名称があり，DenkerはChronische Mittelohreiterung mit randständige Perforationとし，PolitzerはOtitis media chronica cholesteatomatosaと称している。この中耳真珠腫症は発生原因等から原発性と，続発性（二次性）とに分けられ，一般にわれわれがいうものは二次性のいわゆる仮性真珠腫あるいは仮性真珠腫症であり，鳥居氏は耳真珠症と言つている。この病変は1839年Pappenheimが聴器にはじめて発見して以来のものであり，その成因については発見以来，すでに100年以上を経ているのにかかわらず，今なお論争を続けていることである。真性の耳真珠腫が極めて少ないことから，二次性の耳真珠腫がそのほとんどを占めているとすれば，それは一応，慢性中耳炎（殊に化膿性）とは何等かの関係があることは想像に難くない。われわれが臨床でしばしば経験する慢性の中耳炎はカタル性のもの，化膿性穿孔性のもの，そして真珠腫が加はつたもの等に区別されているが，真珠腫症ははたして慢性中耳炎中のどの位に存在するであろうか。これに関する文献は内外を集めると数かぎりないほどであり，臨床のみならず動物実験による発生およびその組織学的観察も決して少なくない。外国文献では，Scheibe，Manasse，Uffenorde，Turner，Rüedi等のものから，発生説に関する古いBezold，Habermann，Wittmaak，Arbrechtのもの，新しくはTumarkin，Friedmann，Diamant，Mc Guckin等の説に到るまで，それぞれ考え方を異にしている。

　A clinicopathological study was conducted on the changes that occur in the middle ear ossicles affected with middle ear cholesteatoma. The results were as follow:

　(1) The degree of bony changes does not necessarily agree with the severity of the disease. (2) Changes appeared to be the strongest in the incus. (3) That the ossicles had been exposed to mechanical pressure mainly due to vascular congestion rather than simple inflammation was evidenced by the way in which atrophic changes were seen in the bones and their invironment. Consequently, in the operation of cholesteatomata the question that whether or not the middle ear ossicles should be removed completely should be a subject of further study.
