Thirteen cases with primary cholesteatoma were treated in the past six and half years. Primary cholesteatoma was diagnosed according to Derlacki 's criteria.

Eight patients had history of acute otitis media and/or otitis media with effusion. Eight patientshad a white mass in the middle ear, two had a perforation of the ear drum. Only two patients showed normal ear drum. These results suggest that Derlacki's criteria for diagnosis of primary choles-teatoma has no longer been appropriate, and it should be diagnosed using the criteria as follows ;

1. No acquired chance of grafting of epithelial cells in the middle ear structure.

2. No connection between the ear drum and cholesteatoma.
