I．はじめに

　髄外性形質細胞腫は形質細胞腫の中で比較的稀なものであるが，上気道に発生することが多い。本邦での耳鼻咽喉科領域からの報告は約100例に及ぶが，最近ではその産生免疫グロブリンに関し詳細な検討がなされるようになってきた。しかし，筆者らの渉猟した範囲では長期間経過観察した報告は少なく，産生免疫グロブリンの消長に関しても下分知られていない。

　今回，筆者らは鼻腔原発と考えられるIgA産生型髄外性形質細胞腫の1例を経験し，約2年半にわたる経過を観察することができたので，若干の文献的考察を加えてここに報告する。

　A 62-year-old man developed extramedullary plasmacytoma in the left nasal cavity.

　We identified and measured the tumor-producing immunoglobulin with homogenized fluid extracted from the tumor. As the result, it was found that the tumor produced IgA and its level was 2400mg/d1.

　In spite of postoperative chemotherapy and radiation therapy, the blood concentration of IgA was maintained at high level. However, signs of multiple myeloma were not recognized during these two and half years. The patient is now under a careful clinical obeservation.
