κ型light chain産生上顎洞髄外形質細胞腫の1例

A Case of Lightchain k Producing Extramedullary Plasmacytoma in the Maxillary Sinus

I．はじめに

　耳鼻咽喉科領域で遭遇する形質細胞腫は，その大部分が髄外形質細胞腫であり，比較的稀な疾患である。我々は最近，左上顎洞より発生したと思われる1症例を経験し，さらに酵素抗体法で腫瘍細胞の産生免疫グロブリンの種類を同定したところ，非常に稀な，light chainのκ型免疫グロブリンを産生していた。また，多発性骨髄腫への進展の有無についても検索し得たので，本症例の臨床像について述べると共に，本疾患の文献的考察を加えて報告する。

　A 57-year-old man with extramedullary plasmacytoma in the maxillary sinus was treated by surgical operation and irradiation.

　After tumor resection, k lightchain was demonstrated in the cytoplasma of the tumor cells by immunohistochemical method (enzymelabeled anti-body method). This immunoglobulin is the first reported in the field of otolaryngology in our country.

　It is very important to distinguish extramedullary plasmacytoma from multiple myeloma, because treatment and prognosis are completely different.

　Detection of the tumor producing immunoglobulin by the enzyme-labeled antibody method is usefull to follow up this disease.
