　I．緒言

　髄外性形質細胞腫は，一般に良く知られている多発性骨髄腫とは異なった臨床経過を示す形質細胞腫として，注目を集めている疾患である。本邦では，現在まで約100例の報告がされており，その多くは鼻・副鼻腔・上咽頭などの上気道に発生するといわれている。最近我々は，左鼻腔に初発した髄外性形質細胞腫に対して，腫瘍切除と放射線治療を行い，良好な経過をたどっていたが，2年6カ月後に右前腕皮下に転移をきたした症例を経験したので，若干の考察を加え報告する。

　A 62-year-old man with complaints of nasal obstruction and nasal bleeding presented a tumor in the nasal cavity which was excised. But the tumor recurred at 6 months, and was treated with irradiation. Two and a half years later a metastatic subcutaneous tumor in the right forearm was extirpated. The patient showed no evidence of recurrence of the tumor at 3【1/2】 years from the time of initial treatment.

　Extramedullary plasmacytoma of the head and neck can well be controlled for long time with local treatment only. Therefore initial curative local treatment is very important.
