上咽頭に発生した髄外性形質細胞腫の1例

A case of extramedullary plasmacytoma of the epipharynx

I．はじめに

　形質細胞腫は形質細胞が腫瘍性増殖をきたす疾患であり，そのほとんどが多発性骨髄腫の形で発症するが，骨髄以外に発生した場合に髄外性形質細胞腫と呼ばれる。髄外性形質細胞腫は形質細胞腫全体の4～5％にすぎず，その約80％が頭頸部領域に発生し，多くは鼻副鼻腔領域のもので占められる^1,2）。今回，上咽頭に発生した髄外性形質細胞腫の症例を経験したので報告する。

　A case of extramedullary plasmacytoma of epipharynx is reported. A 76-year-old woman was referred to us because of epipharyngeal tumor. Physical examination and CT showed a mass lesion in her epipharynx without cervical lymphadenopathy. Biopsy of the tumor showed extramedullary plasmacytoma. There were no abnormal findings in the serum immunoglobulin level,serum and urine protein levels,or bone marrow biopsy. Surgical resection of tumor was performed followed by radiotherapy with a total dose of 50Gy to the upper neck including the epipharynx. No local recurrence or progression to multiple myeloma was noted after 6 months.
