上顎洞髄外形質細胞腫の1症例

Extramedullary Plasmacytoma of the Maxillary Sinus

　I．はじめに

　形質細胞腫（plasmacytoma）は，骨髄を主病変とする全身疾患である多発性骨髄腫（multiple myeloma）として一般に知られている。しかし，形質細胞腫は骨髄以外にも全身すべての網内系より生じることがあり，骨髄以外の網内系より発生した場合には髄外形質細胞腫（extramcdullary plasmacytolna）とよばれ，骨髄には病変を認めない比較的稀な疾患である。本邦においては，1914年吉井^1）の報告以来，現在にいたるまで自験例を含めても102例を数えるにすぎない。

　筆者らは最近，右上顎洞悪性腫瘍を疑い，放射線治療および手術切除を行った症例が病理組織学的に孤立性髄外形質細胞腫であった1症例を経験したので，若干の文献的考察を加えて報告する。

　Primary extramedullary plasmacytoma of the right maxillary sinus was found in a 60-year-old man who complained of nasal obstraction. The right nasal cavity was filled with a yellowish mass. Roentgenographically, the lateral wall of the maxillary sinus was destructed. Histological examination of the mass revealed plasmacytoma and the patient was treated by surgical resection and irradiation of Lineac 5000 rad. During the period of follow-up, neither local recurrence nor evidence of multiple myeloma were found. Extramedullary plasmacytoma may proceed to generalized mye-loma after a certain period of time, thus the follow-up examination should be lifelong.
