Repair of Frontal Bone Defect by Ceramic Implantation

I．はじめに

　前頭洞に対する鼻外開放手術の整復には，骨形成法，骨橋形成法，充填法など種々の術式があり，病態に応じて適当な術式が選択施行されている。しかし，とくに前頭洞嚢腫患者などの前頭洞前壁の広範囲骨欠損を有する症例の鼻外開放手術では，時に術後前壁骨欠損による顔面の局所陥凹が生じ，醜形を呈することがある。

　今回われわれは，前頭洞嚢腫患者の前壁骨欠損に対して，人工材料としてのアルミナ・セラミックスが，きわめて組織親和性がよいという特徴に注目し，これを用い修復しえた症例を経験した。術後1年8か月以上観察しているが，異物反応や感染もなく良好な経過をとっているので，症例を呈示するとともにあわせて前頭洞手術におけるセラミックスの有用性や将来性などについて考察を加えて報告する。

　The tissue compatibility of Al₂O₃ ceramics has been proved by the animal studies. Their clinical use and availability have been reported in orthopedic surgery, thoracic surgery, neurosurgery, oromaxillofacial surgery.

　A 57-year-old woman was admitted in our department for a frontal sinus pyocele with a large bone defect. The sinus was reconstructed with Al₂O₃ ceramic plate. In one year and eight months after the implantation, it was observed in x-ray films that the implanted ceramic plate was well adapted to the bony wall of the frontal region and that the inner surface of the implanted ceramic was covered with tissues just like mucous membrane. The pneumatic cavity of the frontal sinus was existed but small. This material showed the satisfactory results functionally and cosmetically.

　The conclusion, therefore, is that the Alb₂O₃ ceramic implant is a usefull material for the bony wall reconstruction of the frontal sinus.
