Vitamin Aと耳鼻科疾患—乳頭腫（3例），悪性腫瘍（16例）の自験例と文献的考察

Vitamin A for Otorhinolaryngologic Diseases

　I．はじめに

　現在，人間の主要な死因となっている悪性腫瘍に対して行われている治療法は，外科的（手術），内科的（化学療法），放射線照射療法が単独，またはいくつか組み合わされて用いられている。これらの治療法は，主として既に癌腫が成立したものを対象としており，前癌状態を元の正常な状態に戻したり，良性と悪性の境界にあるような腫瘍に対して有効であることは少ない。これは，一度上皮の分化が癌腫の方向に進み始めた場合，現在ではそれを抑制する事が不可能なためである。上皮分化をコントロールし癌化を防止する作用を，癌に対するchemoprevention^1）と呼ぶ事ができる。この作用を有する薬剤の一つとして，最近多くの注目を浴びているのが，vitamin A（vit Aと略）とその誘導体（retinoidsと総称される）である。

　vit Aは，視覚や生殖機能に対する生理的な作用の他に，皮膚上皮の分化促進や角質化防止等の重要な働きを有している。1925年Wolbach andHowe^2）が，vit Aの悪性腫瘍増殖抑制作用について実験を行って以来，多くの基礎的研究と臨床経験が積み重ねられて，天然のvit Aよりも上皮分化に対する作用が強く，副作用も少ない誘導体retinoidが約20種開発された。

　One of the functions of Vitamin A is to control cellular differentiation in many epithelial tissues. Numerous studies have been done on the application of this function for the treatment of the epithelial lesions, especially cancer. Vit. A has, however, a limited usefulness in practice, because of hypervitaminosis A. The investigations have been made to find out some retinoids, analogues of Vit. A, which are more sensitive and less toxic to the epithelium than Vit. A. Etretinate is one of the retinoids. The authors used it for the treatment of widespread papilloma of the nose. The papilloma was remarkably reduced in 2 weeks and disappeared macroscopically in 4 weeks. The curtaive process was very impressive. In addition to papilloma, Etretinate was used with radiation and 5-FU for 16 patients with head and neck cancers (modified FAR treatment), and 2 of 16 patients were treated only by Etretinate. These 2 patients had no recurrence of the tumors for more than two years.

　The mechanism of Vit. A and retinoids has not completely been clarified. The usefulness of Etretinate for the diseases of the ear, nose and throat is discussed.
