アスピリン内服によると思われる小児の大量鼻出血症例

Two Cases of Massive Nasal Bleeding in Children Due to Aspirin Ingestion

　I．緒言

　アスピリンは，鎮痛，解熱，消炎剤として古くから用いられている薬剤である。アスピリンの副作用として耳鼻咽喉科領域では，耳鳴，難聴はよく知られているが，出血傾向についてはあまり知られていないようである。

　われわれは鼻腔前部から出血したにもかかわらず，輸血を必要とするほどの大量の出血をきたし，その原因がアスピリン製剤（小児用バファリン^®：1錠中アスピリン81mg，ダイアルミネート33mgを含む）内服によると考えられた小児の鼻出血症例2例を経験したので報告する。

　A 5-year-old male and a 4-year-old female were admitted to our hospital for emergency because of nasal bleeding, and received blood transfusion. The massive bleeding was considered due to aspirin ingestion.

　Aspirin inhibits cyclooxygenase by acetylation which converts arachidonic acid to endoperoxidesin prostaglandin system of platelets. This effect of aspirin accounts for inhibition of platelet aggregation and release reaction and causes bleeding tendency.

　Otolaryngologists should have enough knowledge about bleeding tendency induced by aspirin ingestion, and should take a history of use of drugs, especially aspirin, from the children who present prolonged nasal bleeding.
